INTRALESIONAL STEROID INJECTIONS

An intralesional steroid injection involves injecting a corticosteroid such as triamcinolone acetonide directly into a lesion on/or immediately below the skin.

Triamcinolone injections are carried out in different strengths ranging from 5mg/ml to 40mg/ml. The choice of strength depends on the type of lesion being treated and the therapeutic indication.

These are some of the common skin conditions for which an intralesional steroid injection may be indicated:

  • Alopecia areata
  • Discoid lupus erythematosus
  • Keloid/hypertrophic scar
  • Hypertrophic lichen planus
  • Lichen simplex chronicus
  • Acne cysts

The procedure

Intralesional triamcinolone is injected directly into the lesion using a fine needle after cleaning the site of injection with alcohol or antiseptic solution. The initial dose per injection site will vary depending on the lesion being treated. Generally, 0.1–0.2 mL is injected per square centimetre of involved skin. Intralesional steroid injections are typically repeated every 4–8 weeks. There is typically no downtime involved and the patient can return to normal activities on the same day as the injections.
